- this form is classified as Colonial because of the reference to the Colonial administration and no inclusion of Commonwealth. All components shown here have however been included despite being used in the post-Federation era;
- the vertical serrations indicate the forms were kept in a book to enable details of each message to be recorded and to facilitate the accounting of the fees.
Prepaid Reply forms were of two types:
Type 1: a form completed by the person originating a message to which a reply was sought.
Type 2: a form completed by a person receiving the original message which contained that message and at least a voucher detailing the prepaid amount.
Generally all prepaid forms were serrated horizontally across the middle so a person or the Telegraph Office could retain the section of most interest to them.
No complete form is known but the images below recreate the nature of the complete (type 2) form given to the receiver of the original message.

Top half - front side.
Used at Kellevie 23 April 1903. This form indicates that a message was sent from Sorell on 11th (April??) at 7:20 pm. The voucher was used to pay the 1/- cost of 16 words back to Sorell from Kellevie on 23 April at 7:25 pm. The recipient who sent the reply has been kind enough to annotate the form twice explaining "Use this when reply is paid elsewhere and we only answer". {Thanks :)} |

Lower half - front side.
Used for a message from S. Townsend at Buckland sent to Mrs. C. Richardson at Kellevie on 15 June 1904. This is the message which originated the process and was given to Mrs. Richardson. Normally the message would have ended REPLY PAID - as in the corresponding form below.
