The inference it is a delivery form is drawn from the words "Message for" which are usually used on delivery forms.

The message to be conveyed by this cable form is - like all the others seen - not related to cable traffic.

It may be that both this delivery form and the transmission form shown elsewhere were printed soon after the re-opening of the cable in 1869. For some reason the forms were not used as intended but delivered to some Telegraph Offices for use as ordinary transmission forms.
